Recent\u00a0results\u00a0published in<\/a>\u00a0Kidney International<\/a> reports,\u00a0led by Professor Edwina Brown and Dr Michelle\u00a0Willicombe from Imperial College London, supported by a Kidney Research UK-Stoneygate research project grant, provide\u00a0important\u00a0insights\u00a0into\u00a0the\u00a0experiences\u00a0of\u00a0older\u00a0patients\u00a0receiving\u00a0a kidney transplant.\u00a0\u00a0\u202f\u00a0<\/p>\r\n

More\u00a0older adults\u00a0are\u00a0receiving kidney transplants<\/h2>\r\n

The number of older people receiving\u00a0a kidney transplant\u00a0is increasing.\u00a0Around 9,500\u00a0patients\u00a0who were\u00a065 and over received\u00a0a kidney transplant\u00a0in\u00a02019\u00a0compared to\u00a0around\u00a010,600\u00a0in 2023.\u00a0\u00a0<\/p>\r\n

Frailty is more common in older patients.\u00a0However,\u00a0there is little research\u00a0into\u00a0how frailty\u00a0could\u00a0impact\u00a0physical and mental health\u00a0for transplant patients. By\u00a0identifying\u00a0these effects,\u00a0patients and doctors would have a better understanding of what transplant surgery could mean for the individual.\u00a0It\u00a0could also inform\u00a0interventions to\u00a0better\u00a0support these patients\u00a0to\u00a0try to\u00a0protect their physical and mental health\u00a0following transplant surgery.\u00a0\u00a0<\/p>\r\n

What is frailty?\u202f\u202f\u202f\u00a0<\/h2>\r\n

With age, the body may\u00a0lose its\u00a0built-in\u00a0reserves across\u00a0different body systems, meaning it is\u00a0less able to recover, or bounce back,\u00a0from\u00a0illness and injury. This is called frailty.\u00a0\u00a0<\/p>\r\n

Identifying\u00a0how\u00a0frailty\u00a0affects\u00a0older people receiving a kidney transplant<\/h2>\r\n

Edwina,\u00a0Michelle\u00a0and the team looked at\u00a0210 patients over the age of 60 who were waiting for a kidney transplant<\/a>. They\u00a0investigated\u00a0whether having\u00a0frailty\u00a0impacted\u00a0mental and physical\u00a0health\u00a0in the months before and following kidney transplant surgery.\u00a0This included\u00a0questionnaires\u00a0to assess\u00a0physical and mental quality of life, symptom burden, treatment\u00a0satisfaction\u00a0and the impact\u00a0of their condition on their everyday\u00a0life.\u00a0<\/p>\r\n

Edwina,\u00a0Michelle\u00a0and the team\u00a0also looked at\u00a0medical data\u00a0for these patients\u00a0to\u00a0see if\u00a0people with\u00a0frailty\u00a0were more at risk\u00a0of\u00a0health problems\u00a0related to transplant surgery itself, such\u00a0as infection.\u202f\u202f\u00a0\u00a0<\/p>\r\n

The majority of\u00a0participants\u00a0reported their\u00a0mental and physical health decreased\u00a0following surgery.\u00a0Participants who were frail\u00a0or\u00a0vulnerable to frailty\u00a0reported worse\u00a0symptom burden,\u00a0mental health\u00a0and depression\u00a0following transplant.\u00a0However,\u00a0for\u00a0most,\u00a0the burden of their symptoms and satisfaction with their treatment\u00a0improved after surgery and reached\u00a0a better level than before transplant\u00a0by\u00a0one\u00a0year.\u00a0\u00a0<\/p>\r\n

Following transplant surgery,\u00a0frail\u00a0and\u00a0vulnerable to frailty\u00a0participants\u00a0were more likely to\u00a0have\u00a0a\u00a0kidney that\u00a0took longer\u00a0to begin working properly,\u00a0which\u00a0can\u00a0mean a temporary return to dialysis<\/a>, and were more likely to spend longer in hospital recovering from surgery.\u00a0<\/p>\r\n

Longer term\u00a0function\u00a0of the transplant,\u00a0measured\u00a0after a year,\u00a0was\u00a0also\u00a0not\u00a0as good\u00a0in\u00a0frail\u00a0and\u00a0vulnerable to frailty\u00a0participants.\u00a0However,\u00a0in most cases\u00a0a kidney\u00a0transplant did\u00a0stabilise\u00a0the declining\u00a0physical health\u00a0of\u00a0people who were frail\u00a0and\u00a0vulnerable to frailty.\u00a0<\/p>\r\n

What could this mean for patients?<\/h2>\r\n

By understanding better how frailty affects outcomes for older transplant patients,\u00a0we can provide\u00a0healthcare professionals and patients\u00a0with\u00a0more information on\u00a0what a\u00a0kidney transplant could\u00a0mean for the individual.\u00a0This could\u00a0also\u00a0inform\u00a0more\u00a0targeted\u00a0support for these patients in\u00a0managing both their\u00a0physical\u00a0and mental\u00a0health\u00a0following\u00a0transplant\u00a0surgery.\u00a0\u00a0<\/p>\r\n

“These results have given\u00a0us\u00a0new insight on\u00a0the many\u00a0impacts\u00a0on\u00a0both\u00a0physical and mental\u00a0health\u00a0for older patients undergoing kidney\u00a0transplant surgery.<\/p>\r\n

“We want patients to have the most information to help them make a decision that is right for them, and as healthcare professionals we want to do our best to support these patients.
